i. Pursuant to the provisions of reference {a), Petitioner, a
former enlisted member of the United States Navy, filed
enclosure (1) with this Board requesting, in effect, that the ©
JPD (alcohol rehabilitation failure) separation code and RE-3F
(physical fitness assessment failure) reentry code issued on 31
August 2012 be upgraded.

2, The Board, consisting of Mr. Zsalman, Mr. Tew, and Ms.
White-Olson, reviewed Petitioner's allegations of error and
injustice on 18 June 2014 and, pursuant to its regulations,
determined that the corrective action indicated below should be
taken on the available evidence of record. Documentary material
considered by the Board consisted of the enclosures,. naval
records, and applicable statutes, regulations and policies.

3. The Board, having reviewed all the facts of record
pertaining to Petitioner's allegations of error and injustice
finds as follows:

a. Petitioner enlisted on 7 October 2009. ° He had no
disciplinary action during his period of service. His
detachment evaluation documents that he failed three physical
fitness assessments in the most recent four years and was not
recommended for retention. Therefore, he was not authorized to
reenlist and was honorably discharged on 31 August 2012, and
assigned an RE-3F reentry code and the JPD separation by reason
of physical fitness assessment failure. The separation code of
JPD shown in his Certificate of Release or Discharge from Active
Duty (DD FORM 214) pertains to Sailors discharged by reason of
alcohol rehabilitation failure. JFT is the correct separation

code for physical fitness assessment failure.
CONCLUSION:

Upon review and consideration of all the evidence of record, the
Board was not persuaded it would be in the interest of justice
to upgrade Petitioner's RE-3F reentry code; however, as he was
not discharged by reason of an alcohol rehabilitation failure,
his Separation code should pe amended from JPD to JFT. 4

RECOMMENDATION:

a. That Petitioner's naval record be corrected to show that
he was assigned a separation code of JFT (physical standards
(PFA) failure), vice JPD (alcohol rehabilitation failure} on 31
August 2012,

b. That no further relief be granted.

¢. That a copy of this report of proceedings be filed in his
record. ,

4. Pursuant to Section 6(c) of the revised Procedures of the
Board for Correction of Naval Records (32 Code of Federal
Regulations, Section 723.6(c) it is certified that a quorum was
present at the Board's review and deliberations, and that the.
foregoing is a true and complete record of the Board’s
proceedings in the above entitled matter.
